(A) a copy of the contract;
(B) contact information for the escrow company or closing agent or attorney
(C) contact information for your real estate agent, if any;
(D) how you (or the owner) holds title; and
(E) the owner’s date of birth, and Tax ID Number (“TIN”) or Social Security Number. If title is held in an entity, please provide the name of entity, date of organization and EIN.
